Electromyography and nerve conduction study (EMG/NCS) documentation must capture both the technical parameters of the studies performed — the specific nerves and muscles studied, the waveform parameters obtained — and the physician's clinical interpretation correlating these findings with the patient's presentation. Both components are required to support the appropriate CPT codes and to establish medical necessity.

What must EMG/NCS documentation include?

NCS documentation must include: the specific sensory and motor nerves studied and the extremities examined; the parameters recorded for each nerve (latency, amplitude, conduction velocity, F-wave latency where applicable); and a comparison to normative values with notation of abnormal findings.

EMG documentation must include: the muscles sampled by needle examination; the findings in each muscle (insertional activity, spontaneous activity including fibrillations and positive sharp waves, motor unit morphology, and recruitment pattern); and the physician's interpretation of the EMG pattern.

The narrative interpretation — correlating the NCS and EMG findings with the clinical history and neurological examination — is the most important element. It establishes the diagnosis, the pattern of involvement (length-dependent vs. non-length-dependent, axonal vs. demyelinating, focal vs. diffuse), and the clinical significance of the electrodiagnostic findings.

How does EMG documentation affect CPT coding?

EMG/NCS CPT codes are determined by the number of extremities studied, the number of nerves studied per extremity for NCS, and the muscles studied for EMG needle examination. Correct coding requires documentation of each nerve and muscle studied — without this specificity, the claim cannot support the billed codes.

The CPT codes for electrodiagnostic studies are: 95907–95913 for NCS (based on number of studies), 95860–95864 for needle EMG (based on number of extremities), and 95886 for complete EMG with NCS. Additional codes apply for autonomic nervous system testing, repetitive nerve stimulation, and neuromuscular junction testing.

Documentation errors — missing nerves, uncounted muscles, or absent interpretation — are the most common source of EMG billing denials and downcodes. Scryber structures the electrodiagnostic report to capture all required coding elements.

How should EMG documentation establish medical necessity?

Medical necessity for EMG/NCS must be established in the ordering documentation and confirmed in the electrodiagnostic report. The referral or ordering note should document the clinical indication — the specific symptoms, examination findings, and diagnostic question that the study is intended to answer.

The electrodiagnostic report should begin with a clinical summary that states the indication, the findings of the history and examination that prompted the study, and then present the technical findings in the context of answering the clinical question.

Documentation that simply lists technical parameters without clinical context — 'NCS of right upper extremity performed; see attached data' — does not establish medical necessity, cannot support prior authorization, and provides no clinical value for the ordering or treating physician.
