Complete neurological exam documentation must systematically address: mental status (orientation, attention, memory, language, fund of knowledge), cranial nerves I through XII with findings for each, motor system (tone, bulk, strength by muscle group with MRC grading), sensory system (light touch, pin, vibration, proprioception by dermatomal and peripheral nerve distribution), reflexes (biceps, triceps, brachioradialis, patellar, Achilles with numeric grades), cerebellar function (finger-nose-finger, heel-shin, rapid alternating movements, Romberg), and gait and station.
For many conditions, additional specialized testing must be documented: pronator drift, plantar responses, frontal release signs, cortical sensory testing, or specific cranial nerve maneuvers.
Each finding should be documented with enough specificity to be clinically meaningful — not just 'reflexes normal' but '2+ and symmetric' or 'right patellar 3+, left patellar 2+, bilateral ankle absent.'
How do examination findings support neurological localization?
Neurological localization — the process of determining where in the nervous system a lesion or dysfunction is occurring — depends entirely on the pattern of findings in the examination. Upper motor neuron signs (spasticity, hyperreflexia, extensor plantar response) localize above the anterior horn cell. Lower motor neuron signs (atrophy, fasciculations, hyporeflexia) localize at or below the anterior horn. Sensory deficits in a dermatomal pattern localize to a nerve root; in a glove-and-stocking distribution, to peripheral nerves.
For examination documentation to support localization, each finding must be precisely recorded — not summarized. A note that says 'motor and sensory intact' contributes nothing to localization. A note that records 'MRC 4/5 weakness in right hand intrinsics with 2+ reflexes at right bicep and 1+ at right brachioradialis, with decreased pinprick from C7 distribution' supports localization to a lower cervical spinal or root process.
This level of documentation precision is what distinguishes specialty-quality neurology notes from adequate but clinically thin documentation.
How does examination documentation affect coding?
Under the current AMA E&M framework, E&M level selection is based primarily on medical decision-making rather than examination bullet points. However, examination documentation remains critical for supporting the complexity of the MDM documented in the assessment.
For procedure codes — EMG/NCS, botulinum toxin injection guidance, nerve blocks — the examination must establish the clinical indication and the specific muscles or nerves involved. For prior authorization, the examination findings must demonstrate the severity and progression of the condition being treated.
