EEG documentation must capture both the technical details of the recording — duration, electrode placement, activation procedures performed, and patient state — and a complete clinical interpretation: the background rhythm, any abnormal findings (epileptiform discharges, focal slowing, generalized abnormalities), and the physician's conclusion about the clinical significance of the EEG findings in the context of the patient's presentation.

What must an EEG report include?

A complete EEG report must document: the indication for the study and relevant clinical history; technical parameters (electrode placement per 10-20 system, recording duration, impedance, filter settings, paper speed or equivalent); activation procedures performed (hyperventilation, photic stimulation, sleep) and whether they were adequate; the patient's state during the recording (awake, drowsy, asleep); and the electroencephalographer's interpretation.

The interpretation must address: the posterior dominant rhythm (alpha frequency, reactivity, amplitude, symmetry); the organization and continuity of background activity; the presence of sleep elements if applicable; any focal abnormalities (slowing or sharp activity) with location and characterization; any generalized abnormalities; and any ictal or interictal epileptiform discharges with morphology, location, and frequency.

The clinical interpretation section must state whether the EEG is normal or abnormal, the nature and significance of any abnormalities, and the correlation with the clinical presentation.

How does EEG documentation affect CPT coding and billing?

EEG CPT codes depend on the duration of the study and the level of physician involvement: routine EEG (95816, 95819), prolonged EEG monitoring (95951, 95953), and ambulatory EEG (95953) have distinct technical and professional component billing pathways.

For prolonged monitoring — continuous video-EEG, long-term ambulatory EEG — the documentation must support the medical necessity for extended recording rather than a routine 20-40 minute study. This typically requires documentation of diagnostic uncertainty about seizure type, refractory seizures not controlled on appropriate therapy, or events of unclear etiology requiring capture for diagnosis.

The professional component of EEG billing — the physician's interpretation — requires a signed, dated report that meets the documentation requirements above. An unsigned or undated report, or one that consists only of a list of technical findings without clinical interpretation, does not support the professional component code.

How does EEG documentation support medical necessity?

Medical necessity for EEG is established by the clinical indication: the specific question the EEG is intended to answer. Routine EEG is indicated for evaluation of new-onset seizure, characterization of a known seizure disorder, or evaluation of paroxysmal events of uncertain etiology. Prolonged EEG is indicated when routine EEG is insufficient to capture the event of interest or characterize the seizure type.

The ordering physician's documentation should clearly state the indication, and the EEG report should state whether the study answered the clinical question. When the EEG is normal in the setting of clinically characteristic seizures, the report should comment on this: a normal interictal EEG does not exclude epilepsy, and documenting this clinical reasoning is important for the patient record and for any prior authorization requests that follow.
